New Patient Information
arrive 15 minutes early for your first appointment, 30 minutes if you
are pregnant, in order for the staff to collect information from you
prior to your visit with the doctor. If you were sent a history form,
please fill it out prior to your arrival. If you have a complicated
medical history, it is helpful to bring (or to have sent in advance)
your old records. If you have special concerns you'd like to discuss
with the doctor at your first appointment, please inform the
receptionist when you make your appointment so that she can allow you
and the doctor some extra time. Please notify the office at least 24
hours in advance if you need to cancel your appointment for any reason.
We will make every effort to reschedule you as soon as possible.
